Job Title: Home Health Nurse (RN/LPN)
Job Description:
The Home Health Registered Nurse intakes the initial evaluations visit to a patient on admission and regularly re-evaluate the patient’s needs, initiates the Plan of Care and make necessary revisions in conjunction with the physician’s orders, develops and implements the Nursing Care Plan, and acts as the case manager and works with other team members to coordinate all health team services to provide well-organized care. Supervision of LPNs and Home Health Aides may be required. The LPN provides nursing care to patients in a home setting and reports to the Registered Nurse changes to the patient. Bilingual/Spanish speaking nurses will be highly considered.

Job Description:
The Home Health QA/Case Manager Nurse evaluates and analyzes the initial evaluations and visits on admission and regularly re-evaluate the patient’s visit notes, orders, Plan of Care and make necessary revisions in conjunction with the physician’s orders, Nursing Care Plan, coordinates all health team services to provide well-organized documentation. Ensuring doementaion accuracy is a must. Managing nurse to ensure timley submission of documents and being a mentor to nurse in the field willbe done regulary.

Job Description (General):
The Therapist (PT,OT, or ST) will review and evaluate the physician's referral and patient's medical record to determine the therapeutic services required, plan and prepare written treatment program based on evaluation, perform tests and measurements, plan and administer programs for patients to restore function, and prevent disability following disease/injury, observe and report response and progress to treatment, instruct patient and family in a home therapy program. Supervision of a Physical Therapist or Occupational Therapist Assistant may be required. Bilingual(Spanish) is a plus.

The Home Health Aide/cargiver will assist other clinicians in the patient's needs to perform skilled and non-skilled services, follow the Care/Service Plan, and works with the case manager and other team members to provide well-organized care. Caregivers typically perfom non-skilled services as a homemaker or companion. Bilingual/Spanish speaking caregivers will be considered.
